Physician Assistant or Nurse Practitioner

Not-for-profit, community-based healthcare organization in Western, MA. You will be seeing patients in their home.

Position requires at least 2 years of NP or PA experience seeing adult and geriatric patients.

Travel Role – a territory along the 91 Corridor that is approx. a 30 mile radius

As a member of the Ready Response Team, you’ll provide short term in-person episodic enhanced primary care and care management to patients with significant medical, behavioral, and social complexities that require intensive clinical support.

This patient need for this team is triggered by an acute event or significant change in condition which necessitates close, short term follow up by an Advanced Practice Provider.

Full time Days – Monday through Friday 8:30 am-5:00 pm. Weekend schedule rotation which includes working Saturday, Sunday, and 2 weekdays. Estimated at 6-8 times per year
